Are picture with e.g. needled tits in those categories?

Yes I would say that needles being stuck in tit would fall under the rule as gore or shock content. Things that show violence's fall in this list. Things like real rape, hanging. real torture, crucifixion, suicide,
strangulation, just to name a few.
